Such a game would have to have a high pricetag right off the bat, then add subscription services on top of that, which isn't really a system people are willing to swallow. Especially because you would have to have so much content done already before you even gave it to players (literally 90 levels worth of content to keep players grinding while you work on more content). It's just not worth the money, time, or effort to do all that on such a scale. The problem is, you have a lot of content you have to make (hundreds of unique raids that offer replay value, thousands of items-and they have to be balanced so each one could feasibly see use by players, AND you have to design an economy/job system that makes it worthwhile to invest time in so that some players would rather do economics instead of adventuring, AND you have to balance all that shit for PVE and PVP).
